How do I file an appeal?

If you disagree with the coverage provided on any claim, you may request a
review of the decision using our voluntary appeals process.
Any submitted appeal should clearly state why you disagree with the initial determination, a letter from your veterinarian with an explanation of the condition/incident, and any other supporting medical documentation. 
Please email with a completed Appeal Form (PDF) and supporting documentation from your veterinarian.
Our goal is to process your appeal as quickly as possible. Typically, we processes appeals within 14 business days however it can take up to 30 days for more complicated medical conditions or pets with extensive medical history.
